If your main website is built on Squarespace, Wix, Shopify, WordPress, or Duda — you can still use Captivation Hub forms to capture leads directly on those pages. All you need is the embed code from your form, and you can paste it into your website platform's custom code area.
This article walks through how to get the embed code and how to add it to each of the major website platforms.
What Is Form Embedding?
Embedding a Captivation Hub form means placing a form directly on your website using a short script. Visitors fill it out right on your page — no redirect needed. Once submitted, the data syncs automatically to your Captivation Hub account so you can follow up through your automations and CRM.
Step 1 — Find Your Embed Code in Captivation Hub
Open the Form Builder
In your Captivation Hub account, go to Funnels in the main navigation, then click Forms in the top menu. This will show your full list of forms.
Choose or Create a Form
Select an existing form to edit, or click + Add Form to build a new one. Once your form is ready, you can grab the embed code.
Copy the Embed Code
Inside the form builder, click the Integrate tab in the upper-right. Choose the type of form from the options, then click Copy Embed Code. Your embed code is now ready to paste into your website.

- From the widget panel, select Widgets > HTML Widget and drag it onto the page.
- Paste your embed code into the HTML widget.
- Save your changes and publish the page.
- Open your site in a browser to confirm the form is loading correctly.
Troubleshooting
If the form isn't showing up after embedding:
- Double-check that you copied the embed code completely, without any missing characters.
- Make sure the code is placed in the correct section (head, footer, or body depending on your platform).
- Try refreshing the page or clearing your browser cache.
- If you're using a popup form, confirm that the trigger is set up correctly inside Captivation Hub.
